The Physical Therapist directs physical therapy treatment toward preventing disability, relieving pain, developing, improving and\/or restoring muscle function and maintaining maximum performance within the client\u2019s capabilities in accordance with the plan or care\/treatment. The PT will oversee physical therapy assistants to assure appropriate implementation of the plan of care. \u2022 Evaluates the client\u2019s level of function by applying diagnostic and prognostic muscle, nerve, joint and functional abilities tests \u2022 Evaluates the home environment for rehabilitation and makes appropriate recommendations to increase the independence of the client \u2022 Supplies physical therapy services in the home as prescribed by a physician \u2022 Administers manual therapeutic exercises; instructs, motivates and assists clients in non-manual exercises and functional activities \u2022 Administers traction, massage and treatments involving the application of physical agents \u2022 Evaluates, fits and adjusts prosthetic and orthotic devices and recommends any modifications to the physician \u2022 Participates in the development, review, and updating of the care plan \u2022 Observes, records and reports any changes in the client\u2019s condition and response to therapy and makes suggestions for changes in the plan of care to the client\u2019s physician and Case Manager \u2022 Prepares clinical records (initial evaluations \u2013 including OASIS, care plans, progress notes, discharge summaries) and submits them for the client\u2019s clinical record \u2022 Participates as a member of the interdisciplinary team in case conferences and in planning and evaluating the total treatment for the client and confers with interdisciplinary team members as needed \u2022 Advises and consults with the client\/family and other agency personnel \u2022 Submits orders and documentation (clinical and payroll) on a timely basis and according to branch policy \u2022 Instructs and supervises Home Health Aides or CNAs\/STNAs, as applicable, in certain phases of physical therapy services they may to the client \u2022 Instructs the client\/family in the use and care of physical therapy devices and on client\u2019s total physical therapy program \u2022 Instructs and supervises Physical Therapy Assistants in certain phases of physical therapy services they may provide to the client \u2022 Supervises the Physical Therapy Assistant minimally as required by applicable state law \u2022 Completes an on-site functional assessment, review and revision of the plan of care and assessment of utilization of community resources as required by client \u2022 Accessible to the Physical Therapy Assistant when a Physical Therapy Assistant is providing treatment to a client \u2022 Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company What we\u2019ll love about you: \u2022 Passion and commitment to clinical excellence, quality and integrity \u2022 Embracing and leading the patient care mission through providing an exceptional patient experience \u2022 Positive attitude and ability to fit in as a collaborative team member for patient care \u2022 Excellent interpersonal communication and patient\/family engagement skills \u2022 Commitment to clinical documentation excellence and quality along with ability to educate and inspire others \u2022 High energy, focus, work ethic, motivation and commitment to lifelong learning and growth \u2022 Flexibility with diverse clients, cultures, and environments allowing you to provide excellent care \u2022 Ability to demonstrate clinical skills in both a collaborative and solo contributor capacity \u2022 Experience with EHR systems and their implementation\/optimization in complex clinical settings \u2022 Proficiency with federal, state and governing agency regulations \u2022 Participates in continued clinical and in-service education, as well as professional training required by law \u2022 Knowledge and comfort level working with technology and enterprise wide support systems and tools \u2022 Successful management of a high volume of work, changing priorities as needed \u2022 Appropriate judgment and critical thinking in decisions that requiring consideration of regulatory compliance, customer service, patient welfare and company interests \u2022 Outstanding presentation, writing and communication skills \u2022 Promotes the agency through positive representation and communication of its services \u2022 Strong ability to be the public face of the organization, and serve as an advocate for Home Healthcare, Home Hospice and other service lines \u2022 Creative thinker who embraces thought leadership \u2022 Capable of making challenging decisions and willing to take responsibility for them. \u2022 Graduate of an accredited physical therapy program \u2022 One (1) year experience as a physicial therapist (acute care\/home care setting) or as required by law \u2022 Current PT licensure (in good standing) in the state(s) of practice \u2022 Current CPR certification \u2022 Valid driver\u2019s license and automobile insurance<\/p>\n
